Across the Northumberland and North Tyneside foot print we have 10 primary care networks with aligned community nursing teams. The aim of these teams is to improve outcomes for people and maximising access to services through integrated working. The teams’ aim is to maintain people within their own homes/communities wherever possible and facilitate earlier discharge from hospital by delivering a more coordinated approach to care. Examples of this approach include close working with secondary care providers to develop virtual wards and supported discharge initiatives.
By working in a more integrated way, the teams aim to reduce duplication and waste by streamlining processes and maximising the use of resources available at a PCN level. The teams are wrapped around GP practice populations and are supported by robust clinical and operational leadership and management. The teams work closely with colleagues in General Practice and the Third Sector organisations. The overarching philosophy and approach places the individual at the centre and proactively maximises opportunities for people to have choice, personalisation and self-management.
