The Hamot Health Foundation is seeking an energetic and compassionate Community Outreach Coordinator to serve as a trusted bridge between healthcare services and the community. This full-time role primarily works Monday through Friday during daylight hours, with flexibility for occasional evenings and weekends to meet community needs. In this role, you will build authentic relationships with community members, earning trust through consistent outreach, empathy, and active listening. By engaging individuals referred through our health plan, you will uncover barriers to care, identify root causes of medical challenges, and connect people to meaningful solutions and resources. This position is ideal for a resilient, people-centered professional who thrives in both proactive outreach and responsive problem-solving—and who is motivated by making a real, measurable impact on community health. Key Responsibilities Engage up to 40 community members per week through proactive outreach to build rapport and trust. Assess individual needs by identifying social, behavioral, and systemic barriers impacting health outcomes. Develop personalized action plans to address root causes and connect members with appropriate resources. Collaborate with internal teams and external partners to ensure coordinated support. Adapt quickly to changing situations, balancing scheduled outreach with urgent or reactive needs. Demonstrate resilience by handling rejection professionally and maintaining a positive, persistent approach. Required Bachelor's degree Three to five years of experience Obtain a community health worker certificate within 1 year of beginning the role Preferred Human Services bachelor's degree Experience in social work or case management Licensure, Certifications, and Clearances: Act 34 UPMC is an Equal Opportunity Employer/Disability/Veteran
