The Oxford County Community Health Centre (OCCHC) serves complex, marginalized populations who face significant barriers in accessing primary care with a full range of wraparound team-based services. We are seeking a:
COMMUNITY OUTREACH WORKER – Woodstock & Tillsonburg
(Full Time Contract – 12+ months)

CANDIDATE REQUIREMENTS:

  • Baccalaureate degree in a health or social science discipline from a recognized university with a minimum of two years’ experience in a community-based service organization, OR, a Social Service Worker diploma or related education and at least three years’ experience in health promotion / community development in a community-based service organization.
  • Registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
  • Demonstrated ability to work with clients presenting with a diverse range of life challenges, particularly addiction & mental health challenges.
  • Strong communication skills in English and excellent interpersonal skills combined with the ability to network and establish relationships with clients, peers and community partners.
  • Demonstrated ability to take initiative and work independently, as well as contribute to effective team-based care.
  • Knowledge of community resources in Oxford County is an asset.
  • Valid driver’s license and access to a reliable vehicle.
  • Proficiency in the use of computers, an electronic medical record and Microsoft Office.

Responsibilities

The Outreach Worker provides support and system navigation services to individuals and their families, informed by a social determinants of health philosophy. They will work in various settings, including on the Mobile Health Outreach Bus (MHOB) throughout the county (i.e. street outreach, at shelter, and at encampments), and in-office at our Woodstock and Tillsonburg sites, to provide the following supports:
· System navigation
· Client advocacy
· Harm reduction supplies
· Basic needs supplies
· Transportation
· Form completion
· Food security
· Case management where appropriate
As part of the OCCHC team, you will also develop and promote health promotion activities, and act as a resource to the OCCHC interprofessional team, programs, and clients. You will work extensively with community partners to fulfil the role.
Compensation & Benefits: Competitive compensation & pay in-lieu of benefits.
Hours of Work: 8:30 – 4:30 daily; 5 days per week; no weekends.
