Are you passionate about making a difference in the lives of patients with palliative and end-of-life care needs? We have an exciting opportunity for you to join our Specialist Palliative Care Service at the Central London Community Healthcare NHS Trust – Hertfordshire Division.
As a Community Palliative Care Specialist Nurse, you will be an integral part of our integrated Cancer, Non-Cancer, and Palliative Care team , working across organizational boundaries to deliver exceptional care. Your role will encompass:
Advanced Clinical Practice
Training and Education
Service Development/Leadership
Audit

  • Provide highly specialist palliative care advice and support to other generalist nurses, care agencies and health professionals including doctors and consultants, to ensure high quality individualised patient care.
  • Use expert skills in communication, clinical judgement and management within palliative care settings.
  • Use and develop specialist skills when dealing with verbal and physical aggression whilst working with patients and carers.
  • Function effectively as an autonomous practitioner working independently not directly supervised and as a member of a multidisciplinary team.
