Range of Pay: IWCP 2 - $41.66 to $51.17 per hour

Characteristic Duties: Under the general supervision of the Commander of Community Paramedicine.
1. Provide primary health care and chronic disease management to identified patient populations in support of the Community Paramedic (CP) Program.
2. Perform patient assessments and interventions in accordance with approved CP Medical Directives, processes, procedures, policies and standards.
3. Complete, in a timely manner, patient charting, perform consultations with CP program physicians when required, and ensure safe keeping of all patient records.
4. Liaise and communicate with patients, family, physicians, community partners and other healthcare stakeholders for the provision of CP care to reduce patient requirements for hospital services.
5. Complete administrative documentation specified by the CP program.
6. Inspect program vehicle and all equipment at the start and throughout the shift to ensure they are in proper working order and in a clean and sanitary condition. Check and ensure the vehicle is stocked at all times for operational readiness.
7. Ensure all medications and equipment is safe guarded, protected and maintained in accordance with manufacturer recommendations and policy.
8. Participate in research development and evaluation of medical devices, practices, and procedures.
9. Participate in mandatory training programs, continuing medical education programs, quality assurance programs and divisional training programs, as required.
10. Participate in and prepare reports on program demographics, opportunities, trends and outcomes, and make recommendations for program improvement.
11. Coordinate and facilitate the delivery of education, quality programs and information for patients, and the public on services provided, as required.
12. Provide mentorship and training of community paramedics.
13. Participate in research initiatives involving Paramedic Services.
14. Deliver public education sessions to different groups and forums.
15. Ensure good public relations standards are maintained with patients, family, physicians, healthcare team members, public, and other government officials or outside organizations.
16. Respond and function as an Advanced Care Paramedic as required in accordance with the Advanced Care Paramedic job description.
17. Participate on committees, work groups, task forces and special projects, as assigned.
18. Manage information in accordance with legislation and corporate standards.
19. Develop and maintain a thorough working knowledge of CGS’s Safety Manual and the applicable Provincial Legislation listed herein.
20. Performs other related duties as required.
