Summary: The Community Pathway Worker, serving as a Co-Lead within the School-based Early Support Program, plays a crucial role in providing case management to families within the community pathway while also supporting the School Outreach Liaison/EBP Lead with school partnership efforts and parent/caregiver support initiatives. This position involves providing individualized support to families, collaborating with community resources, and assisting in school-based partnership efforts to promote the well-being of children and families. Conduct regular home visits and meetings with families to assess needs, identify strengths, and develop individualized support plans in collaboration with families and program staff. Assist the School Outreach Liaison/EBP Lead in building and maintaining partnerships with school staff, administrators, and stakeholders to support the program's goals and initiatives. Collaborate with school personnel to facilitate communication and coordination between families, school-based services, and community resources. Maintain accurate and up to date notes in system. Proper documentation skills are required. Reporting and strong administrative skills. Provide assistances with community resources related to housing, healthcare, education and basic needs. Preform other related duties needed as assigned. Minimum Qualifications: Bachelors in Social Work or similar Human Services field. At least 2 years’ experience in related field. Good interpersonal skills, ability to work with diverse social, economic, and cultural groups. Proficiency in Microsoft Office. Ability to plan and carry out assignments independently.
