Assessment and Care Co-ordination of clients experiencing a range of mental health conditions including but not limited to, Schizophrenia, Bi-Polar Disorder, Personality Disorders, Depression, Anxiety and OCD.
As well as this the team runs a both Depot and Clozapine Clinics, and the person would be expected to cover these as and when required, or potentially take this on as a regular part of their working week.
The role covers a wide geographical area within Mid-Essex, as such a driving license and access to a car is essential for the role. The team is a made up of other CPN’s, Social Workers, Occupational Therapists, Psychologists and Psychiatrists as well as Support Workers and admin staff.
The team hold regular MDT and Business meetings to discuss the ongoing care of the clients as well as the functioning of the team.
To undertake care co-ordination of a caseload of clients.
Carry responsibility for the assessment of care needs, the development, implementation and evaluation of recovery based care plans.
To maintain effective individualised care through assessment of service user and carers problems. Plan, implement and evaluate interventions and clear, accurate and concise documentation of individualised care.
Following analysis of client assessments, plan creatively to meet their needs including the provision of evidence based interventions.
Prioritise and manage workload to ensure the effective management of a defined group of clients with complex needs.
Responsible for working with colleagues in seeking to improve the quality of services provided to our clients/patients.
Participate in the teams duty rota, providing urgent and unplanned care for those under the team who are allocated only to psychology or psychiatry colleagues, or for those whose care coordinators are on leave or sick
