The post holder will be responsible for the co-ordination of care needs, assessment, planning, and implementation of evidence-based care to a defined group of service users and their carers within the service catchment area whilst under the guidance of senior staff. The post holder will provide professional advice and support to other agencies and members of the multi-disciplinary team. The main responsibilities of this role will be to triage, assess, and present assessments to the MDT for the diagnosis of Dementia. You will be supported/trained in delivering the diagnosis and then the management of the case as appropriate to the individuals needs within the pathway and their own journey.
You will liaise with system partners to support care delivery. You will be supporting patients whose disease progression results in distressing behaviours and working with family, carers, and the MDT in supporting the care and treatment of this progression. You will triage, assess, support and signpost patients presenting with a variety of mental health conditions, using all available skills within the team and those within the system to offer the patient the most suitable pathway to support recovery and wellbeing
