We aim to help people to remain safe and well at home and able to maintain the relationships that are important to them.
The service has an intensive support element within it that offers extended hours and weekend interventions to people who are seen by the team.
The service operates 0900 - 1700 hours Monday to Friday
this post covers a large rural and urban area with extensive travel.
