KEY REQUIREMENTS:

  • Current registration with AHPRA as a Registered Nurse
  • Ability to relate to elderly customers with empathy whilst working effectively with other health professionals, service providers and case managers
  • Strong clinical assessment skills
  • Work autonomously with strong communication, organisational and time management skills
  • Enthusiasm for professional growth and development
  • Competency with IDCs, wound care and strategies for living with dementia – desirable
  • Cultural awareness and sensitivity
  • Current Australian driver’s licence and reliable vehicle - both essential

Responsibilities

ABOUT THE ROLE:

We are seeking a Community Registered Nurse to join our friendly and supportive Home and Community Care team in the Bentley area of Western Australia.
Our Juniper Home and Community Care team provides care in the Community setting and the successful candidate will visit customers in their own homes across the South Perth Metro region.
As a Registered Nurse with Juniper Community, you will partner with customers to assess their needs, implement strength-based strategies and evaluate effectiveness. You will conduct clinical nursing assessments, which will include cognitive assessments and care planning. You will work within the broader Home and Community team including allied health, case managers and community support workers. Renowned for our commitment to the delivery of person-centered care and best practice in relation to the provision of quality care, you will be working in a supportive environment that encourages initiative.

KEY RESPONSIBILITIES:

  • Assess and review customer needs in consultation with case managers
  • Clinical Nursing assessments (PAS & KICKA)
  • Cognitive assessments
  • Develop, implement, and evaluate plans of care in partnership with customers and their significant others for both chronic and complex needs
  • Liaise with appropriate health care professionals to deliver quality care to customers
  • Participate in multidisciplinary team meetings
  • Ensure full compliance with organization, professional and aged care standards
