POSITION OVERVIEW

The Community Registered Nurse is responsible for providing high-quality, person-centred nursing care to clients in their homes and community settings. The role involves clinical assessments, wound care, medication management, health education, and supporting clients to maintain independence and wellbeing.

• Conduct comprehensive nursing assessments and develop individualised care plans.
• Provide clinical care including wound care, continence management, catheter care, and chronic disease management.
• Administer and monitor medications in accordance with professional standards and organisational policies.
• Document all assessments, interventions, and outcomes accurately in client records.
• Communicate effectively with clients, families, caregivers, and other healthcare professionals.
• Educate clients and their families on health management, lifestyle, and disease prevention.
• Respond to changes in clients’ conditions and escalate appropriately.
• Support the dignity, privacy, and independence of clients at all times.
• Participate in audits, quality improvement, and compliance activities.
• Adhere to infection prevention and control practices, Work Health & Safety (WHS), and organisational policies.
