  • Ensure thorough clinical assessment, planning, monitoring, and evaluation of care delivered
  • Organise and conduct case conferences and maintain open communications with clients and their families, carers and authorised representatives
  • Effectively prioritise client needs in collaboration with other medical and health professionals as require
