We are looking for a C ommunity Rehabilitation Assistant to join the dynamic and growing Christchurch & Bournemouth Intermediate Care Team. This is an exciting opportunity to be part of a service that is continually evolving to better meet the needs of our local community.
As a key member of our team, you’ll support the delivery of timely and effective rehabilitation for individuals in their own homes and care settings. You will work closely with physiotherapists, occupational therapists, nurses, and other professionals to:
Prevent avoidable hospital admissions
Support patients’ recovery after illness or injury
Promote independence and quality of life
You’ll also contribute to our innovative Community Hospital at Home service, with opportunities to build your clinical skills — including taking bloods, ECGs, and more — as part of our multidisciplinary approach

Provide rehabilitation support to patients in their own homes, helping them regain strength, confidence, function, and independence following illness or hospital discharge.
Work under the guidance and supervision of qualified Therapy and Nursing staff, contributing to individualised treatment and care plans.
Manage a varied and dynamic caseload, supporting patients with a wide range of conditions including orthopaedic, neurological, respiratory, and age-related frailty.
Participate in the delivery of the Community Hospital at Home service, where appropriate, including undertaking delegated clinical tasks.
Maintain accurate records of patient progress and communicate effectively with the wider multidisciplinary team.
Deliver care and support in line with Trust policies and the principles of person-centred care
The Christchurch & Bournemouth Intermediate Care (CBIC) service operates from 08:00 to 20:00, seven days a week

A valid UK driving licence and access to a car are essential, as this role involves regular travel to patients’ homes across the local area
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
