Edmonton Integrated Services (EIS) is currently seeking an individual to fill the position of Community Resource Facilitator (Disability Service Practitioner) in our Community Support Program.

KNOWLEDGE, SKILLS, ABILITIES:

• Ability to confidently provide frontline, face-to-face support with individuals with varying challenges (which range from multi-diagnoses to addictions and homelessness) in all age groups I would say adults as EIS does not provide supports to anyone under 18.
• Excellent verbal and written communication skills
• Intermediate computer skills
• Strong time management and organizational skills
• The ability to work independently
• Flexibility for occasionally changing hours
• Must be able to identify issues, set priorities, and implement an appropriate plan of action
• Proven ability to work collaboratively

-Developing and implement Individualized Service Plans with Individual and their support networks
-Provide effective and flexible casework to individuals accessing our services
-Liaison between other community agencies and professionals
-Maintain Individuals files in a professional manner
-Ensure Agency policies and procedures are being followed
