Responsibilities:
- Administer medications and treatments to patients as prescribed by a doctor
- Provide case management services to individuals with developmental disabilities
- Collaborate with healthcare professionals to develop and implement care plans for patients
- Educate Individuals supported and direct care staff on health promotion, disease prevention, and treatment options
-Provide medical supervision and staff training for all individuals receiving services.
-Schedule all routine and as needed physician appointments and maintain documentation of all appointments and follow up.
- assure continuity of care for all individuals supported.
Experience:
- Minimum of [2] years of experience as a Community Nurse or in a similar role
- Proficiency in medication administration, including knowledge of different routes of administration and dosage calculations
- Strong case management skills, including assessment, planning, implementation, and evaluation
- Familiarity with working with individuals with developmental disabilities and providing person-centered care
