Key responsibilities

• Complete intake, assessments, care and service planning, reviews and other client-related documentation
• Develop and review individualised care and support plans that reflect assessed needs, goals, preferences, funding and service agreements with clear instructions for support workers
• Maintain regular contact and monitor service quality, safety, satisfaction and progress towards the individuals goals
• Respond to changing needs, incidents, hospital admissions and service disruptions
• With consent, liaise with families, representatives, professionals and external providers, coordinating referrals or specialist advice where required
• Escalate unmet needs, service gaps, funding concerns and significant risks
• Liaise with Service Delivery Coordinator to address client and service-related concerns, manage risks and work to improve participant outcomes
• Participate in rotating on-call roster
