Description Community Services Support Professional Location: Binghamton, NY Schedule: Part-time, up to 29 hours per week Pay: $17.50- $18.75 The Community Services Support Professional is responsible for providing direct care through implementation of Habilitation Plans, promoting independence, community integration, and overall well-being. This role focuses on assisting people receiving services in developing life skills, achieving personal goals, and actively participating in their communities. Specific duties and responsibilities include, but are not limited to: Provide supervision and all direct care required to maintain safety and comfort of the person receiving services Transport or accompany people receiving services on trips to community outings, appointments, or recreation activities for the purpose of community integration and exploration Development and planning of inclusive community activities Establish and maintain positive relationships with area businesses Assist people receiving services with completing daily or routine domestic or personal care tasks Foster and promote skills leading to independent living Assist with the development of Habilitation Plans and following these plans for training and assistance Provide training and assist people receiving services to develop, maintain and/or improve desired individual goals, living skills and personal needs as assigned or required Provide supervision to people supported during program hours and during community outings as to ensure their health and safety Complete required documentation in conformance with rules, regulations, policies and procedures, and billing practices Requirements High School Graduate/GED with some knowledge of or experience working with developmentally disabled preferred. Possess a valid driver’s license acceptable to the agency’s insurance carrier
Responsibilities
Provide direct care and supervision to individuals to promote independence and community integration through the implementation of Habilitation Plans. Assist clients with daily living skills, personal care tasks, and transportation to community activities.
