Description

The Community Social Worker will play a vital role in improving health outcomes by addressing the social determinants of health affecting individuals and families. This position focuses on supporting patients in navigating healthcare services, accessing community resources, and managing social, emotional, and economic challenges that impact their well-being.

  • Conduct psychosocial assessments of patients and families referred by healthcare providers.
  • Develop individualized care plans to support the patient’s social and health needs.
  • Connect patients with community resources, including housing, food assistance, financial support, and mental health services.
  • Serve as a liaison between the healthcare team, patients, families, and external agencies.
  • Provide crisis intervention, grief counseling, and supportive therapy as needed.
  • Advocate for patients’ rights and access to care, especially for underserved and vulnerable populations.
  • Participate in interdisciplinary team meetings to coordinate care planning and follow-up.
  • Document all interactions, assessments, and care plans in electronic health records accurately and timely.
  • Track and report outcomes to evaluate program effectiveness.
  • Participate in community outreach, education, and health promotion activities.
  • Stay current with policies, regulations, and best practices in health and social work.

EDUCATION:

  • Bachelor’s Degree in Social Work (BSW) required; Master’s Degree in Social Work (MSW) preferred.

EXPERIENCE:

  • Minimum of 2 years’ experience in a healthcare or community health setting.
  • Experience working with diverse and underserved populations is highly desirable.

SKILLS & COMPETENCIES:

  • Strong knowledge of healthcare systems, public health, and social service resources.
  • Excellent interpersonal and communication skills.
  • Ability to work independently and collaboratively in a multidisciplinary environment.
  • Culturally competent and sensitive to diverse populations.
  • Skilled in case management and crisis intervention.
  • Proficiency with electronic health records and documentation standards.
