COMMUNITY STAFF NURSE | Band 5 £29,970 - £36,483 per annum The post holder will provide excellent skills in palliative care by: Working flexibly as part of a very supportive team within the Dorothy House Community Team Undertake holistic assessments of patients and families that have been referred to the team, this can be in the home, clinic setting or virtually. To provide nursing care that is patient and family centred, acknowledging the needs of individuals and ensuring high standards of care are provided. To develop skills in other community related departments within Dorothy House Hospice Care which are relevant to community services which improve the patient and family experience of Dorothy House. To assess the bereavement needs of the family and refer for follow-up work with the Bereavement Service.
To utilise learning opportunities to enhance the knowledge and skills of health and social care colleagues. This is a Full Time position, working 37.5 hours over 5 days. ABOUT YOU Due to the community based nature of the role, the post holder will need to hold a full UK driving licence with access to a vehicle.
