Description

JOB PURPOSE To work under the direction of the IV Team leader and to be responsible for the assessment and management of the health needs of an identified population and for the development, implementation and evaluation of programmes of care within a range of target groups. To support and wherever possible maintain patients in their own preferred place of care, enabling them to maximise their independence and optimise their quality of life. To liaise with all healthcare professionals and other agencies to ensure appropriate and effective service delivery. To administer IV medication in the patients own home independently in accordance to trust policies and procedures.
At times be expected to support the wider community nursing team. SERVICE DELIVERY To work within the agreed framework for Community Nursing Services To assess, provide and review holistic nursing care, advice and information to patients and carers within the home environment or appropriate care setting in a safe and professional manner using evidence-based knowledge. Undertake routine initial assessments on receipt of specific referrals to the caseload, acknowledging the needs of clients and their carers. Assess, plan and implement and evaluate a plan of care to meet the patients physical, social, emotional and environmental needs, encompassing a rehabilitative/ self-care approach where necessary.
Escalate as appropriate. To provide skilled nursing care and ability to undertake clinical tasks including administration of medication via intravenous lines, venepuncture, cannulation and wound care. Demonstrate awareness of legislation regarding IV Therapy and associated devices and deliver safe care and advice as appropriate within the community setting. Ensure that the clinical area is safe for practice by ensuring that cleanliness is maintained, materials and equipment are ready for use, including clearing away afterwards.
Demonstrate, teach and supervise the nursing care that can be safely delegated to the patient/carer giving support as necessary. Promote clinical excellence and evidence-based approach to care by complying with the agreed Trust policies and procedures and maintaining clinical competencies in line with competency framework. Take an active role in health education of the individual and community promoting self-care and providing both planned and opportunistic care based upon evidenced based information. Accurately record all patient contacts on Emis web and Trust documentation as per MCHFT policy ensuring patient and professional confidentiality is always maintained in accordance with NMC guidelines.
Ensure that equipment and aids necessary for effective care meet the needs of individual patients and are adequately maintained. Demonstrate and explain safe use of equipment for patient and/or carer where required. Participate in the delivery of care involving complex needs and supporting carers and relatives in distressing circumstances. Refer people to other practitioners when needs and risks are beyond ones own scope of practice or require longer term support.
Prioritising own caseload and effectively time managing. To participate in the measurement and monitoring of quality standards as appropriate. To maintain accurate personal and professional records adhering to MCHFT and Nursing and Midwifery Council policy and guidelines. To ensure the most efficient and cost-effective utilisation of all resources (human, physical and financial).
To report accidents/incidents of patients or self which occur whilst on duty and to complete on the Incident reporting system immediately. To participate in any investigations as necessary. To contribute to and co-operate in the co-ordination of off duty and holiday leave with colleagues, ensuring service cover is maintained. Working effectively to support other team members in all aspects of the service.
To maintain all aspects of MCHFT Integrated Governance Strategy and requirements including active participation in appraisal and clinical supervision in accordance with Trust policy. Act at all times in a manner which illustrates respect for privacy dignity and confidentiality. Promote the individual needs of the patient, by acknowledging preferences, rights and choices, respecting privacy and dignity, and by promoting anti-discriminatory practice, alerting colleagues if patient care appears to be disrespectful or discriminatory GOVERNANCE Ensure compliance with Trust policies, procedures and guidelines for self and others, by taking action/alerting senior management team if practice appears to contravene policy, or if concerned about any aspect of patient care. MANAGERIAL/LEADERSHIP Ensure a healthy, safe and secure working environment, ensuring compliance with legal and regulatory requirements, maintaining accurate documentation and reporting any concerns.
To supervise and support students and new members of the team. EDUCATION/LEARNING To participate in the planning and provision of training for pre-registration students to demonstrate the application of theory to practice within the Community placement. To participate in the orientation and agreed induction programmes for newly appointed staff, and those requiring Community experiences. To participate in the training of other Team members and staff as appropriate.
To provide preceptorship, mentorship and clinical supervision to staff as required. To be accountable for undertaking mandatory/essential training as per MCHFT guidelines. To take responsibility for own continuing professional development and performance and maintaining own portfolio in accordance with the re-registration requirement. To identify own development needs in relation to current practice and future plans and setting personal development objectives.
To attend relevant courses/meetings and mandatory in-house training as directed. To participate in professional training/updating programmes to assist service needs/developments. To participate in the Trusts performance appraisal system and to undertake any identified training and development related to the post. Take responsibility for own learning and development by recognising and taking advantage of all opportunities to learn, including full participation in KSF/appraisal, supervision, action learning and by maintaining a professional/personal portfolio of learning.
RESEARCH/AUDIT Participate in the evaluation of current nursing practice and facilitate further discussion and review as appropriate. Identify those elements of current nursing research that may be relevant to the service and assist in their implementation using a questioning and analytical approach to care. Participate in the Trusts research and audit projects as required, ensuring that any beneficial results are implemented on an agreed basis
