We are offering an opportunity for Band 5 NMC Registered Staff Nurses to join our Meir PCN Community nursing team based at Longton Health Centre.
We are looking for 2 Qualified nurses who are committed to providing high standards of holistic care to our patients to join our busy, supportive & friendly team. This is an excellent opportunity for highly skilled nurse looking for a new challenge or looking to return to Community Nursing.
Many people would be unable to live at home without the support they receive from the Community Nursing teams and community services are admitting patients with a more complex acuity than ever before, with increasingly specialist treatment and care being provided by highly skilled staff. Our Community Nursing teams assess, plan and deliver nursing care to patients within their own homes, in line with evidence-based practice.
It is a privilege to care for patients in their own homes, and by joining our team you will be able to experience the difference this makes to our families within the community.
This role includes daytime working, including weekends and cover of early morning shifts starting from 7am when required.
Due to the nature of community working, ability to travel across the patch to provide care to patients is essential.

Qualifications and training
Essential- Degree in nursing or equivalent, you are a qualified nurse with NMC registration.
Practice supervisor training or a commitment to work towards.
Desirable- Community nursing experience. Genuine desire to work in a community nursing team.
