We have a new and exciting opportunity for the right candidate to join Swansea Bay University Health Board’s Primary Care and Therapies Group.
The successful applicant will provide professional, holistic nursing care and support to patients, carers and families within a variety of community settings, delivered with compassion, dignity and respect.
This post will require individuals that can deliver the highest quality care, ensuring evidence based/ best practice and has a commitment to continuous professional development.
Applicants to have excellent communication skills and have a flexible and adaptable approach to changing work patterns. Variety of shift patterns over a 7 day service.

The successful candidates will be required to function as a member of the multi-disciplinary team, to assess individual patient’s care needs and escalate concerns to the correct member of the team. Working as a lone worker responsible for carrying out the delegation of tasks, assessments and evaluations, and escalating concerns as appropriate.
This role involves working within the community, delivering high quality care to a vast range of patients within their own home setting. There would be an expected allocation of work to be complete each shift, this includes visiting patients as a lone worker. The post will also involve transporting equipment such as medical stock and equipment to patients and to health board sites. Therefore, the ability to travel independently between sites in a timely manner is essential.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
