Our team includes nursing, social work, neuropsychology and voluntary sector roles, as well as apprentices, rotations across the acute and community pathway and opportunities for advanced practice.
Over coming years we aim to further bring together a range of specialists working across the fields of stroke and neurology in Dorset, meaning our integrated team will have access to a wide range of specialists with many years of experience.
This is a full time band 3 Community Rehabilitation Assistant post. Our service is a 6-day service and we promote hybrid working with mixed caseload of community, base and working from home;
North Dorset (base at Yeatman Hospital) covering Shafestbury, Gillingham, Sherbourne and Blandford.
West Dorset (base at Forston Clinic) covering Weymouth & Portland, Lyme Regis, Purbecks and Dorchester.
Post is covering West Dorset area and will be based largely at Forston but travel across the county will be expected at times.
Applications from candidates that require current Skilled worker sponsorship to work in the UK are unfortunately ineligible to apply for this position.
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
Community Rehabilitation Assistants support our qualified staff with delivering therapy in physiotherapy, occupational therapy and speech and language therapy as well as working alongside our stroke specialist nurses. The job is varied and rewarding working with patients with stroke and neuro conditions in their own homes.
To contribute to the team’s role of providing Stroke assessment & rehabilitation to community clients under the guidance of qualified practitioners.
To work independently under supervision and in a supportive role as part of the Service
To deliver holistic six-month stroke reviews in a clinic and at home.
We have a programme initiated within Dorset developing communities of practice regarding psychological care and motor relearning, which postholders can join.
