Description General Description To provide a high quality of life for persons served. Manages all services and activities occurring at the Faison Adult Residence location and directs the workflow of CSC’s working at their location. Responsible for teaching new skills and managing behaviors to maximize independence in activities of daily living. Work location is in a variety of community based settings, but primarily occurs in client’s living setting. This position is Essential to the daily operations of the program and individuals in this role are deemed Essential Personnel. Work schedule is Wed - Sat 7:30am to 5:30pm Essential Core Responsibilities Direct Service -80% Assisting persons served to increase their independence and to exercise their rights by teaching skills in various activities of daily living (self-care, care of the household, communication, leisure, etc.) according to formal plans and as natural teaching opportunities arise, using informal teaching techniques/procedures Assisting persons served to reduce maladaptive behaviors and to learn appropriate replacement behaviors by implementing approved behavior support procedures, according to formal plans and as natural teaching opportunities arise Assisting in maintaining the health, hygiene and grooming of persons served by teaching new skills, implementing first aid as needed, health care and hygiene techniques according to instructions or plans and, if properly certified, by administering medications according to stated policies and procedures Providing active treatment by continually engaging persons served in age-appropriate, functional activities, planning and implementing community access activities, and implementing programs to achieve ISP outcomes Transporting clients serves as needed to community activities, following Faison vehicle training and using Faison-provided vehicles Assisting in ensuring the safety, security, comfort and rights of persons served by responding to calls and requests for assistance, and using sound judgment to react to unplanned and emergency situations Ensuring implementation of policies/procedures in unplanned and emergency situations by periodically serving as the designated on-call supervisor, carrying an assigned communication device, and using sound judgement to make decisions Administrative Activities -15% Documenting care/assistance provided, behavior support and teaching techniques implemented and compliance with various health and active treatment standards through accurate and timely completion of progress notes, checklists, records, schedules, and billing information if applicable Completing quarterly reports for all clients serves, delegating as appropriate Maintaining compliance with applicable federal and state regulations, and Faison policies and procedures. If assigned, serving as the Qualified Intellectual Disabilities Professional and liaison for all matters affecting persons on caseload during program evaluation and regulatory reviews. Ensuring ongoing, proper implementation of active treatment and other ISP components by monitoring the activities of other staff, following up when data is inconsistent, incomplete, reveals lack of progress or otherwise needs revision Maintaining a schedule of supports for all clients, editing, revising, and communicating changes as needed Engaging in established channels of communication to support the team Accessing resources appropriately to ensure the health and safety of individuals served Participating in the planning process by attending staff and team meetings and ISP sessions Ensuring well-trained and motivated staff for assigned program location by participating in selection activities, conducting training and ongoing development which includes observations, coaching, providing daily work flow for CSC staff, and assisting with training new staff Attends assigned trainings and implements procedures as outlined in trainings or in training materials; completes all assignments related to assigned trainings Essential Personnel/On-Call Requirements -5% On a scheduled basis, providing on-call coverage. On-call coverage is defined as being available at all times by phone for clients, and in rare instances, being able to be on-site at the Residence if needed. In the event of a weather emergency, staff that are scheduled to work are required to report to work. #ZR Requirements Experience Requirements one – two years of prior experience in a position of similar scope, preferred ability to recognize natural teaching opportunities and act upon them ability to handle all aspects of personal care with sensitivity and caring ability to allay maladaptive (sometimes aggressive) behaviors while maintaining calm and composure flexibility in assigned working locations ability to communicate through verbal and written mediums ability to observe, ask questions, listen actively, and give and accept feedback effectively a high energy level, adaptability, assertiveness, patience and ability to work in a team environment operation of a motor vehicle is required which includes proof of a valid drivers license, an ability to maintain automobile liability insurance as required by state law in Virginia and an ability to qualify as a driver under Faison insurance guidelines Education Requirements High school degree or equivalent required Essential Physical Requirements Lift 50 pounds (and keep back straight while doing so) Bend at all joints fluently Move in and out of and maintain a kneeling position for up to five minutes Reach in all directions Grasp using both hands Move quickly in both confined and open spaces to avoid safety issues Run in order to retrieve an individual who is moving quickly away from staff Maintain balance while lowering to a half squat position
