Description Individual Support and Community Habilitation (ISCH) Caseworker Employment Type: Full time/Part time Work Setting: This position involves in person visits with individuals in their apartments, attending housing or benefit appointments in the community, and completing some tasks remotely. Position Summary The ISCH Caseworker supports individuals living in the community through the waiver program by helping them grow more independent and confident in their daily lives. This role requires in person visits, assisting with paperwork, creativity in engaging a diverse population of individuals served, and flexibility to adapt to changing goals and needs. ISCH Caseworkers assist with housing recertifications, health insurance and benefit maintenance, and daily supports such as transportation, budgeting, and social engagement. As an ISCH Caseworker, you will see the difference your work makes each week as you help individuals build independence and confidence in the community. You will join a supportive team that values adaptability, collaboration, and the belief that small consistent steps lead to lifelong progress. Requirements Key Responsibilities • Provide weekly in person visits to support individuals with their current goals and needs. • Assist with housing and health insurance recertifications, benefit applications, and ongoing benefit maintenance. • Encourage community engagement by supporting individuals in accessing social and recreational opportunities. • Coordinate transportation, including PT1 and approved private services, ensuring accessibility needs are met. • Complete accurate documentation including service notes, consent forms, and required system updates. • Communicate regularly with supervisors and Case Managers to share progress, barriers, and successes. Qualifications • High school diploma or equivalent required; degree or experience in human services preferred • Strong organizational and time management skills • Effective communication skills and ability to adapt to changing needs • Dependable, self motivated, and flexible in balancing visits with administrative responsibilities • Reliable transportation required for community and apartment visits Why Join Our Team
