About the role:

• The Community Support Worker assesses individual participant needs, develops and reviews tailored support plans, and coordinates health, community and NDIS-funded services. The role is a planning and coordination position. It is not a personal-care or disability support worker role. The position is based at Dee Why and requires travel to meet participants, families and service providers.
