Day-to-day responsibilities include assisting people with daily living activities, encouraging social engagement, and supporting access to local resources and services. The role involves monitoring individual needs, documenting observations, and communicating relevant information to supervisors and partner organizations. The Community Support Worker will help coordinate appointments, participate in care planning, and contribute to a safe, inclusive environment where individuals feel heard and respected. Collaboration with families, caregivers, and community agencies is a key aspect of the position.Qualifications Strong Interpersonal Skills and Communication abilities to build trust, listen actively, and interact respectfully with individuals, families, and professionals.
  • Demonstrated experience or interest in Community Support and Community Health to assist people in accessing services, promoting well-being, and staying engaged in their communities.
  • Ability to perform Case Management tasks, including maintaining accurate records, following care plans, and coordinating with external agencies.
  • Basic knowledge of safeguarding, confidentiality, and professional boundaries in a social care or health-related setting.
  • Reliability, empathy, and problem-solving skills, with the ability to work independently and as part of a multidisciplinary team.
  • Willingness to undertake relevant training and professional development; prior experience in care or support work is beneficial.
  • Availability to work flexible part-time hours on-site in Westfield; ability to meet any local regulatory requirements or background checks.
