Support Recovery and Build Stability in the Community At State of the Heart Recovery, community-based care is essential to our mission. Our Community Support Services (CCSS) program provides field-based case management, advocacy, and recovery support to individuals navigating mental health and substance use challenges. As a Community Support Worker, you will help clients achieve stability in housing, employment, wellness, and daily functioning through personalized support and trauma-informed service delivery. This role blends case management, skill development, and community outreach. You will work directly with clients, collaborate with treatment providers, and strengthen each client’s ability to live with independence, dignity, and accountability. What You Will Do Provide field-based case management, advocacy, and recovery support services in accordance with CCSS regulations and organizational policies. Assist clients with accessing resources related to housing, employment, education, medical needs, and community support systems. Develop individualized recovery and service plans that promote independence, accountability, and achievable goals. Coordinate care with treatment teams, behavioral health providers, community partners, and internal departments. Document all services in case management software with accuracy and timeliness. Conduct home visits, community appointments, and skill-building sessions that support client progress. Promote trauma-informed, culturally responsive, and recovery-oriented practices in all interactions. Maintain compliance with all state requirements for CCSS billing, service delivery, and documentation. Participate in team meetings, professional development, and quality improvement activities. What You Will Bring Bachelor’s degree in human services or a related field and one year of case management experience. New Mexico CCSS Certification required or must be obtained within 90 days of employment. Active New Mexico Medicaid/NPI number or ability to obtain within 90 days. Valid driver’s license, reliable vehicle, and auto insurance required due to community-based work. Strong communication skills, professional judgment, and a commitment to trauma-informed care.
