REQUIRED: (CCSS) CERTIFICATE OF COMPLETION IN COMPREHENSIVE COMMUNITY SUPPORT SERVICES AND;

  • Bachelor’s degree in a human services field with one year experience of case management services with target population (substance abuse, mental health or co-occurring population.

OR

  • Associates degree in a human services field with two years’ experience of case management services with target population (substance abuse, mental health or co-occurring population).

OR

  • High School Diploma or GED with three years of experience of case management services with target population (substance abuse, mental health or co-occurring population).

OVERVIEW

We are seeking a compassionate and dedicated Community Support Worker to join our team. In this role, you will provide intensive case management services, developing treatment/service plans, in conjunction with client and other appropriate staff members of State of the Heart Recovery. Your responsibilities will include supporting daily living activities, include crisis intervention, relapse prevention support and emergency services, ongoing psychosocial assessment, family support services, and vocational support.

SKILLS:

  • Motivation to support others in the recovery process and accept all pathways to recovery
  • Sensitivity to individual needs and show respect for a person’s independence and ability
  • Experience with or utilization of Medications for Addiction Treatment
  • Basic computer skills and software
  • Effective de-escalation techniques
  • Motivational Interviewing
  • Engage in 20 hours of training/continuing education annually to maintain CCSS certification.
  • Additional Requirements:
    · Active New Mexico Medicaid Number/ NPI # or obtain within 90 days of employment
    · Valid driver’s license
    · Must have own vehicle and auto insurance to transport clients to and from appointments
    · Cleared criminal background check and signed statement regarding felony indictments and/or convictions.
    Job Type: Full-time
    Pay: $25.00 - $30.00 per hour

Responsibilities
  • Effectively practices under the guidelines of the Comprehensive Community Support Services
  • Supports persons served with vocational needs; resume building and interview skill building.
  • Communicates regularly with counseling team on clients’ support needs, plans and progress.
  • Provides support with housing needs of the persons served.
  • Collaborate with nurses and other healthcare staff to develop and implement effective care plans.
  • Facilitates groups on topics related to substance abuse, vocation skills, life skill building, etc.
  • Work cooperatively with other community resources and referral sources to coordinate services to clients.
    Qualifications:

Certificate of Completion in Comprehensive Community Support Services and;

  • Bachelor’s degree in a human services field with one year experience of case management services with target population (substance abuse, mental health or co-occurring population.
