Schedule: Sunday to Thursday 2:30pm-10pm
This employee will work with assigned young adult clients with a developmental disability who may also have a mental health diagnosis. FASD, ASD and/or physical needs. These individuals will be living in a supported independent living apartment setting that provides supports for activities of daily living, community skills and teaching life skills towards independence. They will provide delivery of direct support services to all clients whether it is within their home or community. Staff will ensure the physical and emotional well-being and safety/security of all individuals, in alignment with YWCA Hamilton’s Mission and Vision Statements.

QUALIFICATIONS

  • Minimum Education: Developmental Service Worker Diploma (DSW), Social Service Worker Diploma (SSW), Community Service Worker Diploma (CSW)
  • Minimum 2-3 Years work experience in SIL or Community Support Program
  • Emergency First Aid/CPR C Certification, NVCI Training, FASD, MHFAC, ASIST Training
  • Case management Experience
  • Ability to work independently and as part of a team
  • Good knowledge of community resources and Aboriginal Community Services
  • Proficiency with MS Office Applications including but not limited to: Word, Excel and Outlook
  • Working from a feminist, anti-oppressive/anti-racist perspective
  • French Language Skills an Asset
  • Must be able to work designated schedule as assigned and be flexible with changing needs of clients
  • Excellent communication skills both verbal & written
  • Must provide a current Vulnerable Sector Police Check or willingness to obtain at own cost
  • Attend and participate in all YWCA Hamilton mandatory training
  • Must be willing to provide a pre- employment medical examination and annually thereafter
Responsibilities
  • In cooperation with program supervisor, other full time, part time and relief staff, will perform all duties and responsibilities in a productive, professional and cooperative manner which will contribute to the overall goals of the department and in keeping with general policies and guidelines of YWCA Hamilton.
  • Ensure the physical and emotional well-being of individuals we support is maintained.
  • Meet QAM Compliance and MCSS Legislation 299/10 Standards or Schedule 20 By-Laws.
  • Ensure annual ISP’s and reviews are completed as assigned for caseload and that goals are realistic and meaningful and reflective of the clients’ needs.
  • Provide and coordinate the linkage of specialized/clinical support services, referrals and advocacy in order to meet individuals’ needs in cooperation with individuals, families, other staff and community service agencies.
  • Provide physical assistance/support as needed by individual in all areas of activities of daily living including but not limited to: pushing/pulling mobility equipment/devices, prompting or assistance with bathing/toileting and hygiene routines, coordination and support of medical/dental/specialists, medication administration, menu planning, grocery shopping, stock-ups, assistance with chores/housekeeping, transportation, finances, socialization, recreation, and programming.
  • All necessary administrative/documentation tasks are performed: ISP development, client file documentation, case notes, accountability forms, behaviour data collection, and programming, checklists, chore lists, or other reports/tasks as assigned.
  • Support services offered will maintain or increase individual’s independent living skills and personal growth.
  • Meet compliance standards set by MCSS
  • Partake in regular team meetings in order to communicate and share information including achievements, progress, concerns and generate solutions
  • Attend on-going professional development and training opportunities offered to program staff
  • Participate in assist in training of newly hired program staff
  • Ensure annual ISP’s and reviews are completed as assigned for residents receiving service and that goals are meaningful and reflective of their needs and wishes
  • To provide and coordinate the linkage of support services, referrals and advocacy in order to meet clients’ needs in cooperation with families, other staff and community service agencies within the support service domains
  • Implement and carry-out individual program plans, Behavior Support Plans and protocols
  • Contribute in the development, planning, implementation of day/evening/weekend community and agency in-house recreation and social activities
  • Promote personal growth and social inclusion
  • Program and agency policies are adhered to and reviewed regularly
  • All necessary administrative/documentation tasks are performed: MAR sheets, data collection, checklists, chore lists, incident reports, or other reports/tasks as assigned in an accurate and timely manner as required
  • Ensure that housekeeping/cleanliness/maintenance of program areas or accommodations standards are met
  • Establish positive professional relationships with staff, individuals, families, and other community service agency providers
  • Ensure YWCA Hamilton workplace health and safety policies and procedures are followed at all times and act to reduce workplace hazards.
