The Opportunity
We are seeking a motivated Community Support Worker to join our team. This role is ideal for someone with at least one year of experience in the community or welfare sector who is passionate about helping people from diverse backgrounds.
You will engage directly with clients, conduct assessments, and connect individuals and families to vital community services. You will also assist in program delivery, documentation, and community liaison. Importantly, the role requires flexibility to travel to regional areas based on client needs.

Key Responsibilities

  • Conduct client intake interviews and assessments to identify needs and create personalised support plans. * Provide direct assistance, monitor client progress, and make referrals to appropriate community, health, and government services. * Assist clients in navigating NDIS services and accessing ongoing support. * Liaise with local organisations and service providers to strengthen referral pathways. * Deliver community programs, workshops, and recreational activities that promote social inclusion and wellbeing. * Advocate for clients’ rights and interests within the community. * Maintain accurate case notes, reports, and documentation for service evaluation. * Travel to regional areas as required to provide in-person services and assessments.

Skills, Qualifications & Experience

  • Minimum 1 year of experience in community services, social work, or related field. * Diploma in Community Services or related qualification (preferred). * Strong knowledge of community resources, welfare systems, and NDIS processes. * Excellent communication, interpersonal, and organisationalskills. * Ability to work independently and as part of a team. * Empathy, cultural awareness, and a strong client-centredapproach. * Advocacy, problem-solving, and time management skills. * Current driver’s licence and willingness to travel regionally.
