Description

We have a superb opportunity to join Complesso CRS (Community Response Service) team as a Community Team Leader supporting individuals with a diagnosis of a Learning Disability, Autism and Mental Health within the community, and within the crash pad.
Roles are to support a new service being provided to the Black Country ICB from the beginning of October. The service is to prevent hospital admission by providing community support for people in crisis and offering alternatives to hospital admission by providing support within the crash pad located in Wolverhampton.

ABOUT YOU

  • Looking for applicants with previous supervisory experience- such as leading a team, conducting supervisions, spot checks, writing care plans and complete initial assessments on new potential service users.
  • Minimum 2 years experience in a community healthcare setting however other experience may be considered.
  • Ability to liaise with senior Health Professionals and coordinate actions with a wider MDT.
  • Knowledge of CQC, DOLs and safeguarding individuals
  • Previously experience supporting people with Learning Disability, Autism & Mental Health
  • Drivers with Business Insurance essential for the role
  • FULL UK LICENCE is essential (no provisional or international)
  • Ability to work independently but report as part of a team
  • Line managing your own allocated staff team.
  • Reliable, honest, open and able to work person centred
  • Looking to support growth of the service across different local authorities
  • Eager to give a positive experience to those who use the service
  • Confident in liaising with health professionals and having difficult conversations
  • Promoting independence, choice and dignity

Responsibilities
  • This role will be supporting those with a Learning Disability, Autism and a Mental Health diagnosis and may include other such diagnoses
  • Permanent full time and part time positions offering an hourly rate of £13.75
  • Hours of work available are 36 hour and 24 hour contracts
  • We have day and night positions are available.
  • Expenses and paid mileage
  • Head office is based in Wolverhampton so will need to be able to travel between ICB hospitals and community settings across Dudley, Sandwell, Wolverhampton and Walsall. This role will entail you to travel between services which includes head office and the crash pad.
  • Responsible for on-call for this service whilst on shift
