JOB DESCRIPTION

Total 247 Care Pty Ltd is seeking a permanent full-time Community Worker to provide community-based and in-home support services to clients in accordance with individualised care and support plans.

Key Responsibilities:

• Provide direct support to clients in their homes and community settings
• Assist clients with daily living activities in accordance with individual service plans
• Support community participation, employment and social engagement
• Monitor client wellbeing and report concerns to management
• Maintain accurate client documentation and case notes
• Liaise with families, carers and multidisciplinary teams
• Assist in implementing and reviewing client support and behaviour programs
• Ensure compliance with NDIS standards, policies and procedures
