Description

This position will be in the community with the mobile H.O.M.E clinic, a multi-agency team that provides health services to people where they are at: shelters, encampments, housing, and in other community settings. The Community Worker will identify and engage individuals experiencing homelessness and at-risk populations, helping them navigate and access services, referrals, and other vital resources
. Working collaboratively within a multidisciplinary team of physicians, nurse practitioners, social workers, community developers and support staff, the Community Worker will advocate for both the Health Centre and the individuals they serve. This is an impactful role that combines outreach, support, and advocacy in a community setting.

REQUIRED QUALIFICATIONS

  • Postsecondary education in social services and/or direct experience providing education/support/counselling to communities at risk
  • Knowledge and understanding of the social determinants of health
  • Ability to work within the harm reduction model of service delivery
  • Willingness to work with people of diverse backgrounds with an inclusive and anti-oppressive lens
  • Valid G classification driver’s license and acceptable driver’s abstract
  • 3 years continuous auto insurance coverage within the last 6 years
  • Comfortability driving the H.O.M.E team vehicles
