As an experienced community nurse the post holder will be embedded in the Guildford and Waverley Community Nursing team assisting in delivering routine planned care such as medication management while having a focus on advice and assessment to those patients who have complex or long term wounds, including lower limb wounds. They will work alongside community nursing staff to ensure skills are gained and maintained, increasing competence across the teams. They will ensure that patients with complex or long term wounds have a comprehensive holistic review and that care plans reflect evidence based practice
