Description

The Comprehensive Harm Reduction Coordinator is responsible for providing Comprehensive Harm Reduction (CHR)services, prevention and outreach services in our community. Provides direction and oversight to the CHR Specialist Responsibilities and Duties include but are not limited to:

  • Coordinates CHR Efforts
  • Supervises CHR Specialist.
  • Actively engages with people who use drugs and individuals who are living with, or at risk for HIV using a collaborative, person-centered approach grounded in harm reduction, trauma informed practice and healing based practice.
  • Distributes and recovers harm reduction supplies and provide education and support on safer drug use, safer sex, HIV, hepatitis C, and STIs.
  • Conducts street-level outreach activities to people who inject drugs in areas identified as high-risk or high need.
  • Works one-on-one with clients to set goals in achieving positive change as they define it.
  • Identifies clients in need and provide appropriate referrals, including assisting clients with accessing support services.
  • Develops and maintains partnerships with local service providers to reach focus population.
  • Provides rapid HIV/HCV testing and follow-up counseling and works collaboratively with testing staff.
  • Provides personalized assistance to clients to help them with navigation to PrEP.
  • Assists and links clients to medical providers prescribing PrEP.
  • Condom distribution in local neighborhoods where high-risk individuals/target populations typically congregate.
  • Performs other appropriate duties as assigned.
  • Completes all necessary documentation and tracking for grant purposes.
  • Complies with all program protocols, including those regarding confidentiality of client information, and HIPPA regulations.

As a nonprofit organization, all staff may be required to support or assist with agency-wide initiatives, events or programs outside of their primary job responsibilities. Flexibility, teamwork, and a commitment to the agency’s overall mission are essential and required for this role.
Education: High School Diploma or GED required. Bachelors preferred.
Qualifications: Proficient completion of “The Facts and Fundamentals of HIV”, prevention counseling services, STI & HCV treatment training, and any relevant trainings pertinent to HIV, STIs, HCV and prevention in primary care within 3-6 months of hire. Accurate physical assessment skills, Good oral skills, Ability to accurately maintain patient medical records and prepare reports, HIPAA compliance, Ability to establish and maintain effective, positive relationships with clients, Ability to maintain current level of medical knowledge at par with standards of practice, Must be comfortable working with LGBTQ+ populations and other underserved populations, Possess clinical problem solving and critical thinking skills, Client-centered advocate who is comfortable with a diverse population, Demonstrated negotiation and conflict resolution skills to effectively and professionally interact with persons of all age, culture, gender, disability, religion, education, and socioeconomic status.
