Job Description:

ACCOUNTABILITIES:

  • To place Recovering addiction clients who are at risk, or homeless, in safe, clean and supportive housing units
  • To reduce the cycle of homelessness in the mental health/substance use population through the provision of supportive housing; so as to improve quality of life through independent living
  • To employ a psychosocial model of rehabilitation that is focused on recovery The Salvation Army – Canada and Bermuda Territory Union
  • To ensure a client-centred approach that is consistent with the client’s identified recovery needs and goals
  • Through supportive housing, to serve as an ongoing community support for clients who are transitioning to independent living in the community
  • Make contact/build rapport with clients who are presently in an addiction recovery program
  • Educate clients about the Addiction Supportive Housing Program
  • Identify/assess needs of clients who desire admission to Homestead’s Addiction Supportive Housing Program

• Advocate for client needs, as required • Mediation, as required

  • Supportive and crisis counselling
  • Identify and assess short and long term goals with client
  • Provide ongoing Relapse Prevention Counselling and support client in their recovery goals
  • Support clients to implement their action plan
  • Provide referrals, as required, to connect clients to appropriate community support services
  • Provide life skills teaching to clients, as required
  • Assist in the transportation of clients to various appointments, as required
  • Maintain accurate documentation of client case load
  • Completion of monthly progress notes with client, so as to determine measurable outcome indicators of the counsellor and the Addiction Supportive Housing Program, determine if client’s goals are being met
  • Conduct unit inspections to ensure health and safety policies are being maintained
  • Attend team meetings as required
  • Report daily with Homestead staff in the Scarborough satellite office
  • Report to Manager of Addiction Supportive Housing Program
  • Case load is approximately 8 clients per FTE, but can be up to 13 at times. Based on budget capacity to provide additional rent supplements, each FTE counsellor may be assigned up to two additional clients upon mutual consent

The incumbent will travel by both public transit and /or personal vehicle (35% travel required)

EDUCATION AND EXPERIENCE QUALIFICATIONS:

Education, Qualifications and Certifications:

  • The successful applicant will have completed an Undergraduate University degree (e.g. BA, BSc, BScN, BSW, etc..)
  • NOTE: An alternative level of education and experience may be accepted

Experience and Skilled Knowledge Requirements
The incumbent must have the following experience before hire:
Minimum 3 years’ experience working in the addiction/mental health sector with marginalized clientele.
Develop an understanding and support for the mission and purpose of The Salvation Army in Canada and its implications as related to position responsibilities.
Experience within housing sector
Individual counselling experience
Case management experience

    Compensation:
    The target hiring range for this position is $22.97 to $22.97 with a maximum of $22.97.
    Placement in the salary range will be based on factors such as market conditions, internal equity, and candidate experience, skills, and qualifications relevant to the role.

Responsibilities

The Homestead Supportive Housing Program addresses the housing needs of clients residing in Scarborough who have successfully completed an addiction treatment program. The provision of supportive housing serves as a continuation of support for the individual to work towards their recovery, while reintegrating into the community and living independently.
