JOB SUMMARY

Community First Services and Supports (CFSS) is a new program replacing the Personal Care and Consumer Support Grant program in Minnesota. CFSS is implementing in a rolling launch as people are assessed and reassessed for CFSS. A key component of the new program is to provide Consultation Services to each person to help them make informed choices about how they want to use their CFSS allocation and build individualized Service Delivery Plans (SDP). Because this needs to be non-biased support, DHS created a new service called Consultation Services, to do this work individually with people.

Responsibilities for the Consultation Services Quality Assurance and Improvement Specialist include:

  • Provide technical support to increase skills of Consultation Service Providers in their development of individualize plans and provide technical assistance to them when the SDPs do not follow CFSS policy
  • Work collaboratively with the CFSS implementation team, divisions across the State including: Disability Services Division, Provider Enrollment and Eligibility, Aging and Adults services, MNIT, Consultation Services providers and people that use services to ensure a timely and successful transition into CFSS from their legacy services
  • Design and implement the DHS Consultation process and workflow
  • Provide Technical Assistance to Consultation Services providers
  • Provide group CFSS information sessions
  • Lead and coordinate other DHS Consultation Services staff
  • Identify and recommend policy improvements
  • Utilize the MnCHOICES application to access data and information

MINIMUM QUALIFICATIONS

2 years professional experience with human services programs infrastructure and the technical solutions/processes that support the systems. Experience must demonstrate the following:

  • Knowledge of and experience with Home and Community Based Services (HCBS) long term services and supports programs, specifically policies related to CFSS
  • Experience working with MnCHOICES application
  • Experience with business processes, federal and state laws and policies pertaining to home and community based services administration.

Bachelor’s degree in Human Services, Public Policy, Public Health or related field may substitute for 1 year of professional experience OR Master’s degree in Human Services, Public Policy, Public Health or related field may substitute for 2 years of experience

PREFERRED QUALIFICATIONS

  • Knowledge of the MnCHOICES application and where to find assessment data to ensure SDPs meet the assessed need of the person and fit within the requirements of CFSS program
  • Understands and is able to simplify and communicate complex policy information and instructions
  • Knows HIPAA requirements in relation to personal health Information (PHI) and associated data access, sharing and security concerns
  • Variety of experiences working effectively with others from different backgrounds and/or cultures
