Position Title: Consumer Family Advocate
Department: Health & Wellness
Site: 93706 Neighborhood Resource Center
Hourly: $19.00-$20.61
PTE Classification: Part-Time PTE/ 30-hour week, Non-Exempt
Report To: Program Manager
Location: Fresno County

MINIMUM QUALIFICATIONS:

  • Degree in social work, human services, child development, or liberal studies
  • Two years’ experience as a Community Health Worker, Peer Support Specialist, Health Educator, Navigation Specialist
  • Experience and knowledge of services and resources in Fresno County
  • Bilingual a plus, preferably in English/Spanish
  • Proficiency in MS Word, Excel, and Outlook Express, as well as data entry
  • Ability to comply with regulations and communicate official information as appropriate
  • Advanced ability to coordinate efforts, facilitate intervention, and maintain records
  • Exceptional organizational skills and sound judgment
  • Valid CA driver’s license and auto insurance
Responsibilities
  • Serve as a cultural broker (promotora) for community members to access mental health services
  • Assess client/family needs utilizing culturally informed approaches
  • Implement identified assessments and intake forms
  • Provide supportive services include linkages, and warm handoffs
  • Educate community on mental health and wellness, stigma reduction and available resources through presentations, workshops, media, and social media
  • Address individual/family concerns and complaints and ensuring follow-up
  • Track all deliverables in the program through logs, reports, databases, etc.
  • Work in collaboration with Department of Behavioral Health and other service providers to enhance and expand awareness and access to available community resources
  • Maintain resource and referral log of community-wide mental health services
  • Communicate services effectively and post notices of services in locations frequented by the target population (such as offices, businesses, social media and others)
  • Participate in community meetings and trainings including mental health first aid, suicide prevention, coalitions, collaborative groups and others
  • Participate in internal weekly case review activities, meetings, trainings, and events
  • Follow COVID-19 safety guidelines including PPE (face mask, gloves, and sanitary procedures)
  • Perform other duties as needed
