REQUIREMENTS:
· Registered as a Pharmacist with the Alberta College of Pharmacists and proficient in the scope of practice of Pharmacists in Alberta (required)
· Authorization to Administer Injections (required)
· Additional Prescribing Authorization (preferred)
· Continuing Care/Long Term Care experience (preferred)
· Kroll experience (preferred)
· Excellent oral and written communication skills
· A respectful attitude and compassion toward others
· Good manual dexterity
· The ability to remain calm and handle emergencies

ROLE DESCRIPTION:
As a Continuing Care pharmacist, you will work closely with a large multidisciplinary team of health care providers to ensure the best possible outcomes for our clients. You will play a key role in medication safety for residents in Supportive Living, Congregate Living, and Community Support Home living environments. You will have the opportunity to work with a team that prides itself in full scope practice, which includes the support of Regulated Pharmacy Technicians, to support and encourage a focus on clinical assessments and interventions by our pharmacists.
KEY RESPONSIBILITIES:
· Maintain ongoing collaboration and communication within the multidisciplinary healthcare team (including but not limited to: nurses, health care aids, GPs, psychiatrists, mental health therapists, case managers) to develop and maintain strong partnerships
· Utilize technology including live medication administration record systems to provide ongoing clinical care to clients and support to their multidisciplinary care team
· Complete medication reconciliation upon client admission and re-admission
· Assist with the development of continuing care medication policies and procedures
· Actively educate and support clients and continuing care staff while providing evidence-based direction to medication and health related needs and inquiries
· Assess new medication orders for appropriateness and safety of therapy
· Appropriately triage new medication orders
· Ensure appropriate and complete documentation of all assessments, interventions, and clinical work
· Work in a team-based model to ensure the timely and accurate processing of medication orders
· Provide direction and leadership to pharmacy team members to ensure the most effective and efficient workflow
· As required, complete initial check of prescription data entry and final check of prepared prescriptions for accuracy and completeness.
