MORRISON CENTER

is seeking a Contracted Adult Case Manager for the Oakland Maine area. This is a Hybrid position with flexibility to work from home and also to have hands on in person engagement. MUST BE IN MAINE!

REQUIREMENTS OF THE CONTRACTED ADULT CASE MANAGER:

  • Bachelor’s degree in any field
  • If you have a degree in an unrelated field, you must have at least 1 year of full-time paid experience in human services.
  • Minimum 1 year of experience working with individuals with intellectual and/or developmental disabilities (DSP, BHP, etc.)
  • Strong organizational, writing, and time management skills
  • Ability to thrive in a fast-paced environment with multitasking and problem-solving responsibilities
  • Previous experience in adult case management is preferred but not required
  • Microsoft Office Suite

Responsibilities
  • Support adults with intellectual and developmental disabilities in achieving their personal goals
  • Develop and manage Person-Centered Plans (PCPs), assessments, and service goals
  • Maintain effective communication and perform research as needed
  • Ensure all documentation is accurate and compliant with Department of Health and Human Services requirements
  • Conduct monthly home visits and maintain open communication with clients and families
  • Perform research and collaborate with service providers to ensure effective supports are in place
  • Serve as a trusted resource and advocate in the community
