Description
As directed by the resident Plan of Care Implements therapeutic, recreational, educational, and social programs for adult individuals with disabilities or illness using person centered planning. Works within the interdisciplinary team setting. Assists with dining room services and the supervision of care and other needs of residents. Reports to the Certified Activity Professional Responsibilities: • In adherence to the resident’s plan of care implements therapeutic, recreational, educational, and social programs to help adult indivuals with disabilities or illness overcome mental emotional and physical disabilities using person centered planning. Coordinates resident’s needs with the interdisciplinary team. • Assists residents with meals, transfers, internal transportation, and other services as needed. Assists residents to community resources and services • Supervises and directs the work of less skilled Colleagues and Volunteers in the performance of delegated therapeutic, recreational, educational, and social activities and other duties. • Documents and communicates residents programming, and supervision of Volunteers, Colleague assignments, and other items as required by policies and procedures and State and Federal Regulations. • Performs other duties consistent with purpose of job as directed. Required: • High School Graduate • Competency Evaluated Nurse Aide
