Yee Hong is one of the foremost providers of culturally appropriate senior care in the country, delivering high-quality resident experience and care to Chinese and other Asian seniors since 1994. The organization operates 805 long-term care beds and 308 seniors’ apartment units, with an additional 224 long-term care beds opening in a new home by June 2026. In addition, Yee Hong offers four campuses of managed care, a hospice residence, and a wide range of community-based senior and caregiver support services. Through this comprehensive network, the organization serves more than 15,000 individuals annually across the Greater Toronto Area.

QUALIFICATIONS:

  • A current member in good standing with the College of Nurses of Ontario; Bachelor’s Degree (preferred)
  • Minimum 2 years relevant clinical experience in oncological and palliative care nursing, including experience in Hospice, palliative care, Hospital or Long-Term Care.
  • Demonstrated commitment to on-going education and professional growth to maintain current knowledge in the changing face of cancer and hospice palliative care.
  • Demonstrated experience in client assessment and patient advocacy.
  • Demonstrated expertise with pain and symptom assessment and management.
  • Evidence of a demonstrated client-centred approach to care and ability to work with families. Physical ability and emotional resilience to provide care to clients.
  • Knowledge of interdisciplinary care processes, commitment to teamwork and ability to collaborate effectively. Knowledge and experience in support services, program planning and evaluation.
  • Effective communication skills, both oral and written.
  • Strong problem-solving skills.
  • Computer literacy required
  • Ability to speak another language (Chinese, South Asian dialect) is a strong asset
  • May be required to travel to different prospective clients occasionally.
  • Effective communication skills in English. Proficiency in another language such as Chinese is preferred
Responsibilities
  • Fosters a culture of teamwork, compassion and respect that creates effective collaboration between the staff and physicians of Yee Hong Peter K. Kwok Hospice, to promote a client-centered approach for decision-making and quality hospice care.
  • Liaises with prospective residents and their loved ones to ensure strong communication and support is available to our clients from first contact onwards.
  • Provides intake and referral information to prospective residents and caregivers calling from the community.
  • Makes community visits to assess appropriate prospective residents and caregivers referred to Yee Hong Peter K. Kwok Hospice, if necessary.
  • Determines suitability and engages in appropriate follow-up in a timely manner.
  • Acts as a resident advocate where needed and desired by the resident and family.
  • Promotes Yee Hong Peter K. Kwok Hospice, in conjunction with the Manager, Hospice, Hospice Medical Director and Executive Director, Hospice, in the community, providing education and awareness within the medical community and palliative and community support sectors.
  • Work with the Manager, Hospice, collaborates with other organizations in the delivery of hospice and palliative care services.
  • Provides management and leadership coverage in place of the Manager, Hospice, during periods of vacation or absence.
  • Implements effective risk management and quality improvement programs, including infection control, occupational health and safety, incident management.
  • Works collaboratively with the other Yee Hong leaders
  • In conjunction with the Manager, Hospice, keeps up-to-date with developments and best practices in the field of palliative and hospice care
  • Participates on committees and special projects and other duties as assigned.
  • Works with Executive Director and Manager, Hospice to determine the need for volunteers to support needed services at Hospice.
  • Works out a plan to recruit and assess suitability of volunteers to develop a pool of volunteers to meet the need of Hospice.
  • Collaborates with Program Manager and/or Program Worker, Volunteer & Services Development, Community & Professional Services Division to plan and assist in training and development of volunteers; and to plan and organize volunteer recognition.
