SUMMARY The Community Support Services Coordinator organizes, implements, and oversees programs that help individuals maintain independence and engage in their community. The role ensures services meet agency and regulatory standards while coordinating service plans, communicating with stakeholders, monitoring outcomes, completing documentation, and providing resource navigation, crisis support, and advocacy. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. Provide oversight of IHS, Respite, Community Mentor, Life Skills, Board of Education, Personal Support, and Private Pay programs across Connecticut. Supervise staffing and schedules, ensure safe and compliant program operations, implement activities, support intake/discharge processes, and maintain agency property. Establish staff schedules within budget and program guidelines; notify Director of needed supervision or schedule changes. Ensure all staff maintain required credentials (PMT, CPR, First Aid, Medication Administration, Abuse/Neglect, etc.). Coordinate staff time‑off and coverage, ensure adequate staffing, and avoid overtime unless pre‑approved. Ensure individuals receive and maintain entitlements (SSI, Medicaid, DSS assistance, food stamps, rent subsidy, energy assistance, etc.). Recruit, license, and maintain Community Companion Home (CCH) providers; prepare complete licensing packets for DDS. Complete and compile licensing requirements including applications, background checks, health statements, home studies, trainings, environmental checks, fire safety inspections, and reference letters. Support individuals with ongoing benefits reviews and entitlement redeterminations. Coordinate relocation of individuals’ belongings for admissions or discharges from CCHs. Arrange transportation with DDS and CCH families as needed. Provide DDS case managers with reports prior to reviews or IP meetings, including goals and team‑identified issues. Assist CCH licensees with person‑centered service plans and monitor implementation. Report individual risk issues to DDS and complete required CCH site visit reports. Conduct twice‑yearly fire safety inspections and ensure compliance with CT CCH licensing regulations. Train licensees on LON‑based Special Support Payments and assist with tracking. Prepare and submit licensing renewal summaries to DDS and collect required forms (Payment Authorizations, Agreements, Water Safety Training). Assist licensees with corrective action plans, collaborating with DDS and the IDT to ensure implementation. Arrange alternative placements when emergencies arise or when a CCH is no longer appropriate. EDUCATION and/or EXPERIENCE Master’s degree or bachelor’s degree (B. A.) in Human Services, Special Education or related field is preferred. Experience working with people with chronic mental health issues, developmental disabilities, acquired brain injuries and other disabilities in a community-based day or residential program required. A high school diploma/GED and at least four years of progressive human service experience accepted in lieu of degree. SCHEDULE 40 hours flex SALARY $21.70/hr, rate increases to $24.13/hr after 6 months.
