SUMMARY

The Data Coordinator II is responsible for overall coordination of internal CORE Network program data collection requirements and external data reporting for LSF CORE Network and other LSF contract requirements and grant deliverables.

EDUCATION AND/OR EXPERIENCE

Associate degree or equivalent work experience. Two years’ experience in medical or behavioral healthcare setting. Data Entry experience and strong organizational skills
Electronic Health Record (EHR) experience a plus; DCF FASASMS XML experience a plus

Responsibilities

ESSENTIAL DUTIES AND RESPONSIBILITIES INCLUDE THE FOLLOWING:

  • Responsible for state data submission requirements to meet contract obligations for managing entity and the state of Florida
  • Responsible for daily maintenance of the CORE Program tracker to ensure all referrals from Nassau County Emergency Management System, Baptist Nassau Fernandina, and Baptist Nassau Crossing are recorded and maintained properly
  • Responsible for ensuring that each new referral is entered into EHR through inquiry, with initial contact dates and demographic information
  • Assists the CORE Peers with administering the Brief Addiction Monitoring Tool (BAM) on all persons served through the CORE program at time of initial contact/admission, and then every 30 days thereafter
  • Responsible for X12 837 EDI file submission to the Department through the Opioid Data Management System (ODMS) provider portal
  • Assists with review of FASAMS Financial Attestation documents in EMR, as needed to ensure eligibility for LSF funded services
  • Assists with running FASAMS Submission Report in EMR and reviewing errors
  • Ensure corrections are made on Client Data Set errors and Treatment Episode Data Set errors?by communicating with Registration Staff, Open Access staff, Clinical staff and medical staff as needed
  • Verify that Outcomes and Performance Measure documents are completed accurately; reports any validation issues to system administrator and/or EMR support
  • Review errors on Service Event Data Sets; notify system administrator and/or EMR support when mapping errors occur that require reconfiguration and/or discover recoding issues with XML report logic
  • Review FASAMS Missing Data report weekly and ensuring corrections, as needed, are made by Clinical and/or Registration staff
  • Reviews and corrects Carisk Data Errors within Carisk system on daily basis
  • Responsible for preparing and submitting various reports to Managing Entity, as required by Contract (IDP Reports; XML uploads to Carisk express)
  • Assists with Data Entry of TANF EZ Worksheets into SAMH TANF Database for new applicants within 3 business days of worksheet completion with client
  • Assists with discharge of TANF persons in DCF SAMH Database after discharge from funding
  • Participates in State data calls and trainings with DCF, LSF and Carisk
  • Other duties as required
