Essential Duties and Responsibilities: Conducts assessments or screenings with clients to evaluate the nature of their presenting problems in order to determine their needs and develop appropriate treatment plan goals. Develops and provides individual treatment plans; monitors, evaluates and documents client’s progress. May have responsibility for intake documentation. Prepares progress notes, event notifications and other documentation regarding clients in accordance with regulatory requirements and company policies and procedures. Monitors case load daily to ensure required documentation is completed timely; including, but not limited to, assessments, treatment plans, case reviews and discharge plans. Provides direct support services to include engaging clients in Independent Living Skills (ILS) activities, health and wellness promotion, personal assistance services, rehabilitation and vocational skills, training and development services, transportation services and crisis management. Facilitates coordination of care to include case review, therapeutic interventions, and client progress in treatment. Participates in clinical staff meetings, trainings and other activities as required. Completes administrative tasks which may include, work orders, copying and faxing. Must have ability to perform First Aid or CPR as necessary and appropriate. Knowledge in use of Electronic Health Records - NextGen a plus. Experience working with diverse populations preferred. Proficient in Microsoft Word, Excel, PowerPoint, Outlook. Minimum typing speed of 25 wpm Excellent oral and written communication skills. This role is a non-driving position. This position is held at one location and does not require travel to various Terros Health centers. May be 18 years of age and with less than two years’ driving experience or no driving experience. Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role. Must pass background check, TB test and other pre-employment screenings. Responsibilities
The Counselor conducts assessments and develops treatment plans for clients, monitoring their progress and providing direct support services. They also participate in clinical meetings and complete necessary documentation in accordance with regulatory requirements.
