Description
Under the direct supervision of the clinic director, clinical supervisor and/or his or her designee, provides counseling services including identifying, screening and assessing patients to determine the need for substance abuse services as well as identifying, screening and assessing patients, families and/or significant others who require counseling services for substance use disorders. Responsible for completing comprehensive assessments including psychosocial, etc.. Responsible for providing on-going orientation about the program and services. Responsible for developing comprehensive treatment plans, including preliminary treatment plans, collaboratively with patient and interdisciplinary team to address needs and short and long term goals. Responsible for supervising patient's urine collection and monitoring patient's substance use/abuse through urine toxicology. Responsible for providing comprehensive case management services, discharge planning, continuing care services, education and referrals to appropriate health and human services agencies to ensure continuity and quality of care. Responsible for completing all statistical reports as required by the hospital, department and regulatory agencies. In addition, the counselor is also responsible for providing community education as dictated by department or programmatic needs in accordance with established hospital policy and Federal, State and City standards including those established by OASAS, FDA, DEA and NIDA. The counselor will also participate in all related functions of the department   1. Experience working with individuals diagnosed with mental illness and/or substance abuse disorders required 2. Experience working in a substance abuse treatment setting required 3. Additional language fluency is preferred 4. Computer literacy required 5. CASAC Required 6. Undergraduate / Bachelor's Required Pay: 1199 Collective Bargaining Rates
