Job Details Job Location: Haven for Hope of Bexar County - San Antonio, TX 78207 Job Shift: M - F 8am - 5pm SUMMARY. The Contract Crisis Clinician is primarily responsible for providing Living Room Crisis Counseling to single adults experiencing homelessness and living with complex challenges related to trauma, substance use and mental health during evenings and weekends. ESSENTIAL DUTIES AND RESPONSIBILITIES. Primary duties involve assisting clients accessing the Living Room during evening and weekend hours who are experiencing mental health crisis. The Clinician is responsible for conducting crisis assessment. The clinician is responsible for conducting Living Room Crisis Counseling sessions and interventions based on assessment. The Clinician collaborates with the client to write a Crisis Response Plan as needed. The Clinician is responsible for arranging follow up care, including follow up evaluation for involuntary hospitalization as indicated by the client’s presentation. The Clinician is responsible for providing individual, and group therapy and clinical case management services as assigned. The Clinician is responsible for documenting Living Room sessions through case notes, and inputting service provided. As needed, collaborates with clients and clinical team members in the formulation of comprehensive, person-centered recovery plans that are reviewed and modified with the client and team on a regular basis. Conducts initial and ongoing assessments, collaboratively with clients and clinical team, to guide ethical and targeted service delivery. The Clinician works closely with Haven team members, consulting and collaborating on client’s progress, recovery, and treatment plans. As needed provides outreach to clients with complex needs, utilizing pre-treatment engagement and intervention to assist in improving functioning. The Clinician assess clients’ needs and level of functioning Write safety, behavioral, and treatment plans Provide collateral parties with information to best meet the client’s needs as necessary including coordination with Sigma and the Center for Healthcare Services, medication management, prescription resources, and any services that involve continuity of care for clients with mental health and/or substance use disorders. Write progress notes for each therapy session and mental health service. Complete appropriate mental health documentation, including but not limited to, client plans performance outcomes, progress reports, health questionnaires, and termination reports as required. Attend clinical, and team meetings to consult with clinical and program staff. Attend weekly supervision meetings with therapy supervisor if unlicensed, to comply with licensing requirements Collaborates with clients to assess and increase engagement and motivation for change. Provides wellness education Assess, and links resources for life skills education, and vocational rehabilitation Provides service coordination and assists with referrals to appropriate supports to ensure each client’s recovery, wellness, and housing goals. Supports client and helps link clients to appropriate level of substance abuse, medical, and mental treatment Links to, educates about, informs of and advocates for client’s rights regarding benefits and entitlements. Collaborates with clients to assess and increase engagement and motivation for change. Provides wellness education Assess, and links resources for life skills education, and vocational rehabilitation Provides service coordination and assists with referrals to appropriate supports to ensure each client’s recovery, wellness, and housing goals. Supports client and helps link clients to appropriate level of substance abuse, medical, and mental treatment Links to, educates about, informs of and advocates for client’s rights regarding benefits and entitlements. Protects client confidentiality, as specified in agency policy, and in compliance with HIPAA. Complies with professional code of ethics and adheres to the agency mission by valuing individual dignity, empowerment, and a holistic healing approach. Other duties, as assigned. Qualifications EDUCATION. · Minimum Required – Master of Social of Work or Counseling CERTIFICATES, LICENSES, REGISTRATIONS. · Preferred Licensure as an LPC-Associate, LPC, LMSW, LCSW, or LMFT is required. EXPERIENCE. · 2 Years of direct practice with individuals experiencing challenges related to homelessness, poverty, trauma, substance abuse/mental health treatment, physical health, family relations, and access to resources. Direct practice experience to include interactions with community resources to meet diverse needs of clients. CLINICIAN KNOWLEDGE/SKILLS/ABILITIES. • Commitment to Haven for Hope’s mission and guiding frameworks of service delivery (1) Trauma Informed Practice (2) Recovery Oriented Practice (3) Person Centered Practice (4) Harm Reduction (5) Goal-sustaining Recovery • Skill to accurately diagnose and assess clients utilizing DSM-5. • Skills in evidence-based therapy and therapeutic interventions • Ability to document biopsychosocial assessments and treatment plans as required by program • Culturally competent skills in engaging a diverse population • Skills with providing reflective supervision to support team members experiencing vicarious trauma and compassion fatigue • Ability to review, learn, follow, and adapt evidenced based treatment programs to deliver to diverse client groups. 3) Motivational Interviewing • Patience, creativity, flexibility, compassion, and sensitivity to persons with disabilities and underserved populations. • Excellent communication skills, written and verbal, to represent Haven for Hope and support clients with histories of homelessness and traumas. • Excellent administrative ability, to include organizational skills, attention to detail and strong computer skills • Ability to be self-directed and to take initiative, which includes ability to discern when to seek supervision • Strong creative and critical thinking skills, to include the ability to analyze problems, identify alternative solutions, and project consequences of proposed actions. • Ability to work collaboratively and be invested as a member of the assigned H4H Team. • Ability to work evening and weekend schedule.
