Description
The Crisis Coordinator is responsible for coordinating and providing direct crisis intervention services, establishing and maintaining strategic relationships with emergency departments, and ensuring seamless service integration for individuals experiencing mental health and substance use disorders. Essential Functions: Receive and process referrals to determine the appropriate level of care. Ensure receipt of proper referrals, including tracking supportive documentation of the prospective client’s mental health treatment, core history, and prior hospitalizations. Review referrals for both routine and urgent cases, formulating the appropriate level of care. Assess and reassess risks, including suicide or self-harm, for all referred clients. Collaborate with referral sources and other professionals as necessary to ensure comprehensive care. Monitor a caseload of high-risk clients and facilitate access to wraparound services. Develop partnerships with community organizations, healthcare providers, and other stakeholders to enhance service delivery and access. Document and maintain records of collaborations with community partners to support service enhancement. Work collaboratively with the CCBHC Interdisciplinary Team and external agencies/stakeholders. Attend all required management and staff meetings and complete CCBHC and organizational trainings. Meet on a weekly basis with supervisor to discuss achievements of program target, client goals, quality of care and reporting requirements Meet data metric requirements, quality of care expectations using social work interventions and evidenced-based models Perform other work-related duties as assigned, adapting to the needs of the agency. Supervisory Responsibilities: Supervise and participate in student intern program, as applicable. Minimum Qualifications: Master’s Degree Ability to plan and carry out assignments independently 1+ years of experience with adolescents and adults with mental health and/or substance abuse needs. Ability to communicate effectively and efficiently Ability to prioritize adhere to timelines and multi-task Pass clearances, fingerprints, and demonstrate satisfactory health check-up. Preferred Qualifications: LMSW and clinical licensure in Social Work or clinically related field with diagnostic privileges Bilingual in one or more of the following: Spanish, Mandarin, Cantonese, Bengali or other depending on neighborhood needs. Experience in providing crisis and intervention services in a mental health treatment environment.
