Job Details Job Location: Gateway Open Door - Astoria, OR Position Type: Full Time Salary Range: $26.78 - $34.95 Hourly This position qualifies for a $500 sign on bonus, half paid with the first paycheck and half after six months of employment. Clatsop Behavioral Healthcare has moved most positions to a 4 day 9 hour work week with paid 30 minute lunches to promote a better work life balance. Clatsop Behavioral Healthcare (CBH) has an immediate opening for a full time Community Support Services (CSS) /Assertive Community Treatment (ACT) Substance Use Disorder (SUD) Case Manager. The primary purpose of this position is to work with the most acute and highest level of needs of individuals with severe and persistent mental illness. ACT provides support to individuals who are diagnosed with a severe mental illness and whose needs have not been well met by more traditional mental health services. They are identified by high utilization of hospitals and correctional facilities. The employee in this position will work and collaborate with the ACT and CSS multi-disciplinary teams to deliver integrated services that meet recovery goals and align with the OCEACT Fidelity Scale. The ACT team delivers services to individuals in community settings; allowing individuals to be met where the individuals live, work, and socialize. Specific to Open Door case management are service planning and coordination, family and social relationships support, referral to substance abuse treatment and support, housing assistance, referral to supported employment and education, wellness self-management, problem solving, skill development, (including basic personal care and safety, grocery shopping and cooking purchasing and caring for clothing, household chores, using transportation, assessing other community resources), health education and support, medications access, financial management, and empowerment and self-help. Essential Functions/Major Responsibilities Interview clients to obtain information concerning medical and psychosocial history or other pertinent information All CBH employees are Mandatory Abuse Reporters per State of Oregon, so must be aware of and follow Mandatory Abuse Reporting Policies Observe clients to detect indications of abnormal physical or mental behaviors Plan with clients and provide services such as behavior modifications, skill training, education on symptoms and symptom management, and stress reduction treatment, to assist clients in understanding and controlling symptoms and other interpersonal problems. Provide sensitive and assertive community outreach to clients and natural support providers of clients by visiting them in their homes, workplaces, or in the community. Change method and degree of treatment when indicated by clients’ reactions as outlined in the Recovery (Treatment) Plan Discuss progress toward goals with clients, clients’ family, and other natural support providers. Consult with supervisor, team lead, medical director, nurse practitioner, primary care Physician and other specialists concerning treatment plans and make recommendations to the overseeing Qualified Mental Health Professional (QMHP) regarding updates to Recovery (Treatment) Plans. Conduct relaxation exercises, facilitate skills training groups, teach life skills, and other evidence-based services. Refer clients to supportive services to supplement treatment and counseling. Maintain clients’ charts in keeping with Oregon Administrative Rules (OARs) and agency quality assurance standards, protocols, procedures, and timelines. Participate in investigations, hearings and other legal proceedings as directed by agency and/or subpoena and maintain required documentation for the same. Exercise a high degree of understanding and compliance with accepted standards of client care. Understand and support the mission of the agency and adhere to all agency policies and protocols. Consult and coordinate services with other community partners such as: Developmental Disabilities, Primary Care Physicians, Department of Human Services (DHS), Senior & Disability Services, Courts, Community Action and School. Obtain releases of information to be able to consult and coordinate care with other community agencies. Participate in regular team and agency meetings. Complete and maintain all clinical documents and client files according to compliance standards with the OARs and Quality Assurance standards established at CBH. This includes but is not limited to the timely completion of progress notes. Maintain client confidentiality and adhere to ethical and code of conduct standards of the profession. Coordinate admissions and discharge planning with all treatment providers. All ACT team members are responsible for 24/7 crisis coverage for ACT clients on a rotating basis. Other duties as assigned and within the scope of role. Education/Experience: Bachelor's degree in Social Work, Behavioral Health, Psychology (Trauma informed care) or an acceptable combination of education and experience to be certified as a Qualified Mental Health Associate (QMHA) Two years prior working experience with people diagnosed with serious mental illness. Valid Oregon or Washington Drivers license Excellent oral and written communication skills Strong attention to detail, highly organized and ability to multitask. Knowledge of Health Insurance Portability and Accountability Act (HIPAA) and importance of maintaining confidentiality. Ability to learn and adopt the policies and procedures of the agency. Ability to pass criminal background check. Desired Skills: Knowledge of DSM-5 Knowledge of civil commitment process Experience with electronic health records Knowledge of Trauma Informed Care Skilled at remaining calm in stressful situations and de-escalation techniques.
